Case of the Week 26

A farmer presented with bloody diarrhea and abdominal cramps. Endoscopy revealed multiple ulcers, and the following biopsies were obtained:

I think this one is hard. It looks like there are some red blood cells inside it, and I guess it could an E. histolytica trophozoite, but it is kind of big. It also has a vacuole in the cytoplasm, a bean-shaped nucleus, and I think I might be seeing cilia all around, so maybe it could also be Balantidium coli?
